Let's look at the second question. The original sentence is, The CD single to which you are listening is one of my favourites. We need to rewrite it using that.
Question 2
Original: *The CD single to which you are listening is one of my favourites.*
Requirement: Use that
When we replace which with that, we move the preposition to the end of the relative clause. So, it becomes, The CD single that you are listening to is one of my favourites.
Moving on to number three. We have two separate sentences: He is the doctor. He operated on me last month. We need to join them using who.
Question 3
Original: *He is the doctor. He operated on me last month.*
Requirement: Use who
Since the doctor is the person performing the action, who replaces the subject he in the second sentence. The final result is: He is the doctor who operated on me last month.
