Step by Step Written Solution
In this exercise, we need to assign the appropriate nursing precautions for five different clients based on their symptoms and medical needs. We must choose between Airborne, Contact, Droplet, or Standard precautions. Remember, Standard precautions apply to every patient.
Nursing Isolation Precautions Analysis
First, we establish that Standard Precautions are the foundation of healthcare and apply to all patients regardless of their diagnosis. This protects both the nurse and the client from blood and body fluids.
Looking at the table, we've already marked Standard Precautions for all five clients. Now let's determine if any additional isolated precautions are needed based on clinical presentations.
Let's analyze Client number one and Client number three. These patients often have conditions like MRSA or VRE that are spread by touch. Contact Precautions require gown and gloves.
Client 1 and Client 3
Diagnosis suggests pathogens spread by direct or indirect contact.
As we see in the table, both Client 1 and Client 3 are marked for Contact Precautions in addition to their Standard precautions.
| Client | Precautions |
|---|---|
| Client 1 | Contact + Standard |
| Client 3 | Contact + Standard |
Now let's look at Client number two. This client's symptoms or diagnosis suggest a highly infectious disease that can travel long distances through the air, such as tuberculosis or measles.
